Insurance Billing

Do you want to use your medical insurance to pay for your massage sessions?

We would love to help you get going on this benefit.

And as a courtesy to you, we offer insurance billing directly to your insurance company!

We are preferred providers with Regence, Bridgestone Insurance, American Specialty Health, and Aetna Insurance.

We also accept Workman’s Compensation and Auto Injury/PIP claims. If Kiva Therapeutics is not a preferred provider with your health care plan you may be able to submit a receipt to the healthcare company and be reimbursed for your out of pocket expenses.

How do you know if your insurance covers massage?

You can log into your insurance member home page and look at the benefits description.  Or give your insurance company a call  to see if you have coverage of massage therapy benefits. Unfortunately Medicare does not cover massage therapy. You may have massage benefits in your secondary insurance. Or give us a call and we would be happy to set up a treatment plan for you to address your needs.

You will also need a prescription for massage therapy in order for us to bill your insurance.  This can be obtained from your family physician, orthopedist, chiropractor and physicians assistants. Please have the doctor email or mail a prescription to:

Paperwork:

We will need you to complete some forms before we can bill your insurance. This will allow us to receive payment from your insurance company.  You can find those forms HERE

We will need copy of your insurance card to put in file for billing.  We can take a copy of it when you arrive for your first visit.

Please understand that you are responsible for all dates of service regardless of determination by your insurance company. If your insurance company denies coverage, you are still responsible for provided services.

Co-payments and co-insurance payments are due upon date of service after the initial billing period and copay determination.
