Insurance Billing
Using Insurance for Massage Therapy
Would you like to use your medical insurance to pay for massage therapy?
As a courtesy to our clients, Kiva Therapeutics offers direct insurance billing to many insurance providers. We are happy to help you determine if your massage therapy sessions may be covered.
In-Network Insurance Providers
Kiva Therapeutics is currently a preferred provider with:
- Regence
- Blue Cross / Blue Shield
- Premera
- Kaiser Permanente
- Bridgestone Insurance
- Cigna
- Aetna
We also accept:
- Workers’ Compensation claims
- Auto Injury / PIP claims
If Kiva Therapeutics is not currently in-network with your insurance, you may still be able to submit your receipt to your insurance company for out-of-network reimbursement.
We are also happy to apply to join additional insurance networks. If your insurance is not listed, please call us and we can check whether joining your plan is possible.
How to Check If Your Insurance Covers Massage
The easiest way to confirm your coverage is to:
- Log into your insurance member portal and review your benefits, or
- Call the member services number on your insurance card.
When speaking with your insurance company, ask the following:
- Does my plan cover massage therapy?
- Is a prescription or referral required?
- Does the massage need to be paired with chiropractic care or another provider?
- How many visits are covered per year?
- What is my copay or coinsurance?
- Does my deductible apply?
Medicare
Unfortunately Medicare does not currently cover massage therapy.
However, you may still have massage benefits through secondary or supplemental insurance plans. If not, we are happy to work with you to create a cash-based treatment plan that supports your health goals.
Required Paperwork
Before we can bill your insurance, a few items are required.
1. Insurance Forms
Please complete our insurance intake forms prior to your first appointment.

2. Insurance Card
We will need a copy of your insurance card for our records.
You may bring it with you to your first appointment and we will make a copy.
3. Prescription for Massage Therapy
Insurance companies require a medical diagnosis in order to bill for a date of service.
Massage therapists cannot diagnose conditions, so this documentation must come from a licensed medical provider. We will need a referral or prescription for massage with an ICD-10 code in order to complete the billing process.
Prescriptions can be written by:
- Medical doctors (MD/DO)
- Chiropractors
- Physician assistants
- Nurse practitioners
- Orthopedic specialists
- Other prescribing physicians

There are many ICD-10 codes that your medical practitioner may prefer. But these are a great place to start in order to explain what is required.
Payments & Insurance Responsibility
Please note that patients are responsible for all services received, regardless of the final determination by your insurance company.
If your insurance denies coverage, payment for the services remains the patient’s responsibility.
Copays & Coinsurance
Copayments or coinsurance amounts are due once your insurance has processed the claim and the patient responsibility has been determined.
Initial Visits
For new insurance claims, we may request 50% of the cash visit rate for early appointments until insurance payments are established.
These payments may then be:
- Applied to your deductible
- Applied to your copay or coinsurance
- Refunded if an over-payment occurs
